A Clinical Study on Respiratory Care for Adult Respiratory Distress Syndrome after Abdominal Surgery






Abstract
In recent years the applications for artificial ventilation after surgery have broadened with improved diagnostic and therapeutic tools, and better understandings in pathophysiology of adult respiratory distress syndrome(ARDS). This report includes fifteen cases of ARDS complicated after various kinds of abdominal surgery, which are compatible to 2.295: among total numbers of the patients(681) who operated the department of general surgery of Seoul National University Hospital during six months from October, 1975 to March, 1976.
The following results were obtained;
1) Male to female ratio is 11: 4.
2) Average age of ARDS after abdominal surgery in SNUH is 51.4 years of, age with 53.5% of mortality rate, which reveals no differences in sex.
3) More frequent incidence and higher mortality rate of ARDS after abdominal surgery noted below 6.Ogm% of total ¢¥serum¢¥ protein and 2.9gm% of serum albumin.
4) Average durations of artifical ventilation are 35.6 hours in survived group and 44. 5 hours in dead group.
5) Preoperative ventilatory values(minute volume, frequency and vital capacity) and arterial blood gases reveal no differences between survived and dead groups except larger vital capacity in surved group than in dead group.
6) Six cases among fifteen cases of ARDS transfused more than l0 points units of acid-citrate-dextrose stored blood.
7) Possible occuring date of ARDS after abdominal surgery is any time during postoperative¢¥ periods.
